At Edward Waters University, the Sports Medicine Department is committed to providing top-tier healthcare for our student-athletes. Our team consists of nationally certified athletic trainers, holding Board of Certification (ATC) credentials and Florida Board of Athletic Trainer Examiners (LAT) licensure. These professionals play a critical role in injury prevention, evaluation, immediate care, rehabilitation, and reconditioning, ensuring our athletes stay healthy and perform at their highest level.

Beyond injury management, our Sports Medicine team handles essential administrative duties such as medical record-keeping and facilitating insurance claims, while also serving as a vital link between coaches, student-athletes, families, team physicians, and Athletics Department administration to coordinate the best possible healthcare for EWU's athletes.

Recognizing the importance of a drug-free sporting environment, EWU—in alignment with the NCAA—conducts randomized drug screenings to promote the integrity of competition and safeguard the health of our student-athletes.

EWU SPORTS MEDICINE FORMS
Each year, every student-athlete must have a pre-participation physical evaluation from a licensed doctor turned into the athletic training staff clearing them to play sports. Contact Student Health Services to schedule an appointment or call your family doctor to get a physical.

Please have your doctor use the Pre-participation Physical Evaluation form provided here. Any form filled out by an outside physician must have that physician's name, phone number, and address clearly printed by the signature or include an attached business card.
The sickle cell tests must include the following information for them to be accepted by the Sports Medicine staff. The document must be a results of a clinical laboratory test. The result must include your name, DOB, Date of collection, Lab information, referring physician details, and results of the screening. We will also accept newborn screenings with the above information. WE WILL NOT ACCEPT LETTERS FROM A PHYSICIAN or SCREEN GRABS FROM MY CHARTS.
All prospective and returning student-athletes must complete the following forms and be returned to the athletic training staff each year before participating in any work-outs, practices, or games with any intercollegiate team. No EWU student or high school recruit may participate in any team-sponsored physical activity, including try-outs, without first submitting a Medical Packet to the athletic training staff
